Requesting Transcripts
You may request an official copy of your transcript by downloading the form from the link below. Upon completion you can return it to the Guidance Office in one of the following ways:

You may also do a request via phone with Mrs. Matteson, guidance secretary at 941-5336. If you are connected with Mrs. Matteson's voicemail please leave your name that you attended high school under, the last year you attended and the complete address to send the transcript.

Allow at least 48 hours in advance of the scholarship or college application due date in order to allow for sufficient processing time. Official transcripts are mailed directly to an institution. Only unofficial transcripts will be given to the individual.
