Transferring to MCHS
Transfer Students
If your son or daughter is transferring to Morris Community High School, there are procedures that need to be followed to insure a smooth transition.
A registration packet must be completed by picking it up in the main office. The main office can mail you the documents that need to be completed prior to registration. Please do not just walk in to register. The following item MUST be presented before an appointment can be made with a counselor:
* Morris Community High School Student Information Sheet - detailing student contact information.
* Affidavit of Residence - This applies to a student who is not living with a parent or legal guardian. A Power of Attorney must be provided from the person who the student is living with who is not a natural or adoptive parent or been granted custody or short-term guardianship.
To complete the Proof of Residency and Custody the following must be provided at the time of registration:
* Proof of Custody
Category 1 - (all documents required)
( ) Student's Birth Certificate
( ) Legal custody papers if the student does not live with both birth parents. If living with a birth parent who does not have legal custody, you must provide a notarized letter from the other parent granting permission for your student to live with you and attend MCHS as well as a copy of the legal papers.
( ) Parent identification (driver's license or state I.D.)
* Proof of Residency
Category 2 - (one document required)
( ) Mortgage papers
( ) Real estate tax bill
( ) Real estate sales agreement
( ) Lease
Category 3 - (two documents required showing proper address)
( ) Driver's license with new address
( ) Voter registration
( ) Gas or electric bill
( ) Checking account or bank statement
( ) Home/apartment insurance papers
( ) Credit card bill
( ) Public aid card
( ) Furniture transport documents or forwarded mail
* School Records
Current high school students (To be obtained from your prior high school)
( ) Unofficial high school transcript
( ) Illinois State Board of Education Student Transfer Form (if you are transferring from an Illinois public school)
Eight grade students enrolling at the beginning of their freshman year
( ) Copy of grade school diploma
( ) Freshman physical is required before the first day of school
